Important Back-to-School Information Please click on the following link to read important information for parents about preparing for the start of the 2010-2011 school year. Race and Ethnicity Letter and Form A new race and ethnicity form must be completed for each student for the 2010-2011 school year for the State of Illinois. One copy of the form is included in the Back-to-School packet. Additional forms may be downloaded here.
